One minute you’re biting into your lunch without a care in the world, and the next, something feels… off. Maybe there’s a slight wiggle, or perhaps you hear a faint clicking sound when you press on it.

Here’s what I want you to know right away: Yes, a loose dental implant can often be saved, but timing matters. A lot. The sooner you address it, the better your chances of keeping that implant exactly where it belongs.

So when an implant feels loose? That’s your body waving a red flag.
But here’s the thing: what feels loose might not actually be the implant itself.
Sometimes it’s the crown (the tooth-looking part on top) or the abutment (the connector piece between the implant and crown) that’s wiggling around.
Other times, it’s the titanium post in your jawbone that’s lost its grip.
Understanding which component is loose makes all the difference in whether we can save it.

What's Actually Loose? Breaking Down the Possibilities

1. The Crown or Abutment Is Loose (Good News!)

This is probably the best-case scenario. The screw that holds your crown to the implant can loosen over time, especially if you grind your teeth at night or chew on particularly tough foods. You might notice:
Can we save it? Absolutely. In most cases, I can tighten or replace the screw, and you’re good to go.
Sometimes we need to replace the abutment or adjust your bite to prevent future loosening, but the implant itself stays put.

2. Bone Loss Around the Implant (More Serious)

If the bone supporting your implant starts breaking down, the implant can become unstable. This might happen because of:
Can we save it? Maybe. If we catch it early enough, we might be able to treat the infection, improve your oral hygiene routine, and even perform a bone graft to rebuild support.
But if the bone loss is extensive, removal might be necessary.

3. Infection (Peri-Implantitis)

Peri-implantitis is basically gum disease for dental implants. Bacteria sneak under your gumline, causing inflammation and eventually attacking the bone. Warning signs include:
Can we save it? Often, yes, if you see me quickly. We’ll need to deep-clean around the implant, possibly prescribe antibiotics, and address whatever’s causing the infection. In some cases, a bone graft helps restore lost support.

4. Failed Osseointegration (The Implant Never Fully Bonded)

Sometimes the bone simply doesn’t fuse with the implant properly. This can happen if:

The good news? We can often place a new implant after your bone heals, possibly with a bone graft to improve your chances of success.

What I Need You to Do Right Now

If your implant feels loose, here’s your game plan:

4. Keep it clean. Gently brush and floss around the area. If it’s too tender, rinse with warm salt water instead.
